Freely Living OCD and Anxiety Therapy is a fee-for-service provider and does not accept insurance.
As of 2026, Dr. Alyssa Hoffman is no longer in-network with any health insurance providers. Becoming an out-of-network provider was not an easy choice; however, I am hopeful that it will allow for better care and quicker progress for those I work with.
While I could discuss the variety of reasons mental health providers choose to work out-of-network, the ones that are most relevant to you include a more available clinician and more flexibility with your treatment plan. By reducing the administrative load of accepting insurance, I am able to dedicate more time to my ongoing education, management of treatment plans, and coordination of care. I am also able to carry a smaller caseload, offering more flexibility for rescheduling, more time for other tasks, and more intensive treatment upfront when needed.

Acknowledging that every family has different financial resources, health benefits, and supports, the following options may be available to you.
1. Out-of-Network Benefits - You will be provided with a monthly “superbill” that includes a diagnosis, service provided, fees paid, and all of the business information you need to submit to your insurance. If your benefits include out-of-network benefits, you will be reimbursed directly the amount your insurance plan covers. For some plans, your allowed fees will contribute to your out-of-network deductible prior to receiving any financial reimbursement. If you wish to call to verify coverage, the codes most commonly used are 90791 (Initial Appointment) and 90837 (Therapy - 53-55 minutes). There are also online programs that help streamline this process. If you need support with this, please don’t hesitate to ask.
2. Network Override / Network Gap Exemption - If you have tried in-network providers or searched and searched and come up with no one that will be the right fit, your insurance plan may have to admit that there is a gap in their network. When this happens, a similar process to using out-of-network benefits occurs; however, insurance reimburses you based on your in-network policy. It is not a simple process to prove there is a gap in an insurance network; however, when this is true and you need specialist treatment, it can be worth it.
3. Sliding Scale Spot - Dr. Hoffman maintains a set number of sliding scale spots in her calendar. Should you need a fee reduction, please ask to be sent the sliding scale form. Based on your current financial situation, you may qualify for a reduced fee.
4. INESA - Dr. Hoffman is an approved provider for the Indiana Education Scholarship Account. Based on your specific needs, sessions may be payable in full through this account.
5. Referral - There are times when the financial piece may limit our ability to work together. That does not mean you shouldn’t have access to appropriate care. If you need assistance in finding an in-network provider, you are welcome to reach out to ask for a list of names. While I cannot verify the experience or care provided by these clinicians, it may help you in reducing your search time. The IOCDF also maintains a resource directory of providers where you can filter by insurance.
